From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23)During a practice autorotation the right tail rotor pedal broke. All flight controls were functioning normal until the end of the maneuver. At the bottom of the maneuver after a slight flair and level off, the nose of the helicopter began to yaw left and the pilot applied right pedal. Approximately three feet off the ground a "pop" was heard and the pilot felt the right pedal break under his foot. The pilot managed to land the aircraft safely with no damage to the aircraft and no injuries to the crew. The police department suspected possible metal fatigue of the pedal and submitted a FAA form 8010-4.
