From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) On July 9, 2001, allison hoffman (dob 11/05/1978) sustained fatal injuries from a skydiving incident. Several skydivers jumped form 13,500 feet AGL fro N220EA, a DHC-6-200 (twin otter) aircraft. Three skydivers had seen ms hoffman at 5,000 feet, in freefall position, in control of her descent. At approximately 2300 feet her main parachute was deployed. The steering toggles indicated she had pulled them from their stowage during descent. The main parachute cut away handle, which released the main parachute from the harness, was with the body at impact. Its deployment indicated use. The automatic activation device (aad) functioned and released the reserve pilot parachute. The pilot parachute was inflated but entangled in a tree at the site. The main parachute was approximately 23 yards from the body. The reserve parachute was outside of the container, but had not deployed completely. The reserve parachute had a pack date of February 14, 2001. Ms. Hoffman had made a previous jump on this date.
