From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Immediately prior to this incident Mr. Silva had been giving instrument flight instruction to Mr. Jamie chubb, holder of private pilot certificate no. 2634832. Due to adverse weather Mr. Silva and Mr. Chubb elected to land at traidwinds Aerodrome, scottsmore, Florida. After the weather improved they attempted two takeoffs--aborting on each occasion. Mr. Silva determined that he was going to attempt a third takeoff. Mr. Chubb elected to exit the airplane and not be a party to any additional takeoff attempts. On the third takeoff attempt the aircraft (at rotation speed) veered to the left, leaving the runway. Mr. Silva stated that he... "Reduced power but the aircraft continued to the left going into a ditch on the left side of the runway". Mr. Silva further stated that he felt that the left main tire hit a "pot hole" or some sort of depression causing the airplane to veer to the left. The runway was wet at the time of the incident. On inspection by the FAA it appeared that the runway had not been mowed for several months.

As the FAA filed it
