From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) The pilot attempted to take off from Cavanaugh Bay Airport at 0630 am local time and ran across some bare spots in the grass after a run of approximately 100 feet. Right after passing these 1.5" deep but flat spots, the gear unsafe light came on and the right main gear collapsed, followed by the nose gear. He said he walked the area after the incident and saw propeller strikes in the grass ahead of where the right main gear failed and thinks the nose gear linkage failed and initiated the collapse. Investigation revealed that the nose gear foreward left hand push pull rod end bearing failed. Also, the nose gear truss assembly right hand attachment for the right hand push pull rod failed at the truss end of the tubing cluster weld joint. It appears the gear system was not maintained to it's original or properly altered condition as required by FAR 43.13 (b).

As the FAA filed it
