From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Investigated incident involving a skydiver which resulted in a fatality: on July 18, 2001, around 2110 (8:10 pm) a skydiver was fatally injured when he was performing a normal (free fall) jump from an aircraft, his main parachute deployed from its pack but did not open up. He successfully cut the main chute away and proceeded to deploy his reserve chute. According to witness he was in a spin when he deployed the reserve chute, this caused the chute to become tangled thus failing to open. There were no other injuries.
