What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
(-23) With no intention for flight the pilot was conducting the second high speed taxi test of his newly built experimental airplane. At APPROXIMATELY40 mph the pilot added full throttle. The nose rotated shortly thereafter, at which time he retarded the throttle to idle. Very soon after the throttle was retarded the left main and nose gear collapsed. The plane then slid off the runway to the left.
