(-23) At approximately 23:50 EST on 17 November, 2001, Mr. Steve c. dickinson, holder of pilot of certificate number 522784837, operated his Piper PA-28-161, N85TT on an instrument flight plan (IFR) from the Deland municipal airport (ded), Deland, Florida to the merritt island airport (coi), merritt island, Florida. Shortly after takeoff from runway 5 at the Deland municipal airport, at approximately 150 feet altitude, the engine failed. The engine failure is attributed to carburetor icing. Mr. Dickinson attempted to turn back to the airport looking for an open space to land his aircraft. The aircraft crash landed in a wooded area consisting of scrub oak trees and palmetto plants. The aircraft sustained major damage with non-life threatening injuries to the pilot and his passenger. The passenger, Mrs. Helen c. dickinson sustained head injuries and was removed from the aircraft in an unconscious state. Meteorological conditions were marginal VFR with a temperature and dew point conducive to carburetor icing conditions. privacy data omitted
