The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) After departing from a hospital pad, at approximately 200 feet the pilot heard a loud bang from the rear of the aircraft. The aircraft then started to yaw to the right. The pilot made an approach into a cow pasture. After landing the left skid contacted uneven terrain and left the ground. After regaining ground contact the nose of the aircraft began to tuck. The main rotor blade then struck and severed the tail rotor driveshaft. There were no injuries.
