What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
(-23) Right main landing gear collapsed on landing. Cockpit indications showed "unsafe" retraction and "safe" gear extension for landing. Forward pivot trunion moved forward because the securing roll pin appeared to have failed. Gear slipped from the trunion support and was no longer secure on landing. No injuries. (.19) on June 18, 2002, approximately 1950 pacific daylight time, a Cessna 441, N564AC, experienced a right main gear collapse while on landing roll at rogue valley international airport, Medford, Oregon. The aircraft was being operated by erickson air-crane. The airline transport pilot, and his three passengers were not injured. However, the aircraft sustained substantial damage. The 14 CFR part 91 business flight, departed lake tahoe airport, south lake tahoe, California, approximately one hour and twenty minutes prior to the accident, and was being operated in visual meteorological conditions. An instrument flight plan had been filed and activated. According to the pilot, after takeoff the gear retraction did not feel normal, and it appeared as though the gear cycle did not end. The pilot re-cycled the gear and noted that both the gear transition light and the hydraulic light failed to extinguish at the end of the cycle. The landing gear circuit breaker was pulled to turn the hydraulic pump off after the gear handle toggled to the up position. Passengers visually confirmed that the right main gear was not retracted. Prior to landing the pilot observed three green lights on the gear position indicator sub-panel. During the landing roll, the right main gear collapsed. Upon initial post-accident inspection, it was found that the right main gear trunnion roll pin had sheared.
As the FAA filed it
(-23) RIGHT MAIN LANDING GEAR COLLAPSED ON LANDING.
