From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Thomas terning was the operator of an amateur built sky scout aircraft that was departing his private airfield. During take-off, the aircraft cleared the trees at the south of end of the runway, but impacted the ground approximately 25 feet beyond the trees and to the left of the runway. The aircraft crashed in a near vertical attitude, moving only approximately six feet after impact. The pilot received critical injuries and is mentally incapable of providing any pilot information or any aircraft operating hours. His spouse does not feel comfortable or certain and has chosen not to provide any information about the pilot or aircraft. There were no witnesses of the accident.
