From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) On March 19, 2003 at 06:55 local time, Mr. Dennis f. bonner, certificate number 2736223, the pilot in command of N9450C, a PA-34-200T, attempted a departure on runway 17R, at David Wayne Hooks airport (dwh). Approximately 300 feet down the runway the right wheel came off the pavement. The pilot initially reduced power on both engines, however, when he was unable to control the aircraft he added power on the left engine in an attempt to get back on the pavement. His attempt failed, and the left propeller hit a concrete culvert and the aircraft came to a stop just north of taxiway delta, with the left main gear deeply buried in mud. The pilot informed us he had been distracted by an oil pressure gauge moving erratic during the initial takeoff run.
