What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
(-23) Aircraft N28UP was being flown by PIC edward rick mize (PIC) who was participating in the wings weekend program conducted at greenwood, IN airport (hfy). Pic's company owns N28UP. Douglas jenkins (CFI) was providing non-compensatory instruction to PIC. On Monday, June 2nd both PIC and CFI were interviewed separately. According to interviews the CFI briefed, and both PIC and CFI understood, that mize was the designated PIC. Additionally, mize signed a statement that he would be PIC. Both PIC and CFI departed hfy in N28UP and proceeded to accomplish air work after which the aircraft proceeded to bmg for multiple full stop landings. After two successful full stop landings accomplished by PIC, a third landing was made by the PIC. However, the landing gear was not extended and N28UP landed gear up. N28UP does have an automatic landing gear extension system installed. All of the air work and the approaches and landings were accomplished with the automatic landing gear extension system in the disengaged/deactivated configuration, as permitted by the POH. The time at which the automatic system was disengaged/deactivated and the reason for doing so was not an issue to which both PIC and CFI agree. CFI did state that PIC was consistent with following checklist procedures during the entire flight and did remember that PIC had verbally accomplished the gump check prior to contacting the ground. However, CFI stated that he had not visually confirmed the illumination of the green landing gear lights and/or the position of the landing gear lever prior to the gear up landing. Layout of the cockpit and instrument panel places the green 3 landing down, the amber auto gear extension light, and the landing gear lever just to the immediate left of the engine control console. This layout does not provide easy visual observation of the landing indication system by the occupant in the forward passenger seat.
