From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Medevac flight from salem hospital to altman hospital, crew of 3 and 1 patient. Shortly after take-off the #1 engine lost power, and the PIC shut down affected engine and performed a precautionary landing in a cow pasture located at 4450 paris ave., Washington Twp., OH, without further incident. No injuries were sustained and the aircraft did not sustain any damage. Subsequent investigation into engine failure revealed the power turbine (pt) of this engine had seized. The pt has been tagged and sealed and is going to be sent to the engine manufacturer for evaluation. All required reports (mrr, SDR) concerning this incident have been submitted by the air carrier, to this point.
