From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Amendment to follow. (-23 amendment) on June 15, 2003, Mr. Ken tolliver, a private pilot, crashed N4TD, a Cessna CE-337, at the Beacon Star airport near lewistown, Montana. According to Mr. Tolliver, when he landed at the airport, he used full nose up on the electric trim during the landing flare. After landing and prior to his next take off, he failed to recenter the trim. During his next takeoff, Mr. Tolliver rotated the aircraft, which became prematurely airborne. With full nose up trim, he was unable to control the aircraft. It stalled out of ground effect and departed the runway to the right. The aircraft came back down to the ground and rolled approximately 1,200' in a wheat field. The aircraft then struck a bar ditch and slid across a road. The tail boom assembly broke and the right main gear separated from the aircraft. The aircraft sustained substantial damage, but the occupants of the aircraft were unhurt.
