From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Evergreen international airlines (eiaa) June 25, 2003 #1 engine, fire and in-flight shutdown. 06/25/2003, in flight shut down/diversion to jfk airport aircraft N471EV engine s/n 689564 aircraft departed dover air force base enroute to frankfurt; during climb crew reported an #1 engine fire light illumination. Discharged fire bottles, EGT red balled and nacelle indicators indicated red zone. Shut down engine, dumped fuel and diverted to jfk airport. Preliminary investigation reveals that #1 engine, #4 bearing oil pressure supply line broke at the "b" nut base, discharged oil ignited onto hot case which started fire. Noted during inspection of engine: several clamps attached to oil supply line were found broken and not attached, in addition clamps were found without grouments inserts which may have allowed vibration of oil supply line. Preliminary damage: aft exhaust sleeve, sustained large burn holes at the 2, 6, and 9 o'clock positions, holes in excess of 2 square feet. Metal was noted in tail pipe: burn damage to wiring and lower case, aft of gearbox. At this time no additional aircraft damage has been noted. Aircraft tt, 87384 hours - 20984 cycles; engine tt since new 67015HOURS - 15173 cycles; engine TSO, 5652 hours 1204 cycles; overhaul, by ge aircraft engines, wales. Last c check airframe/engine, 04/16/2003, 587 hours/131 cycles. NTSB, lab inspection of defected line revealed that failure of line was due to approx 10% fatigue at line breaking point, in addition, improper clamping of line caused severe vibration which aided in line failure.
