From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) On Friday September 5, 2003, at 18:30 local time, an unregistered and uncertificated amateur built avid flyer aircraft, operating under 14 CFR part 91 on a local pleasure flight collided with terrain shortly after takeoff from a Private turf airport located at 7745 kendall road in Pavilion, NY. According to the certificated Private pilot, he was providing flight instruction to his neighbor, a non-pilot and owner of the aircraft. Both believed the aircraft was an ultralight and that they were operating under the rules of 14 CFR part 103. The pilot stated that shortly after clearing the treeline following takeoff, the aircraft suddenly made an uncommanded pitch down and roll to the left. The aircraft struck the ground shortly thereafter. The pilot was seriously injured. The passenger was uninjured. Both occupants were utilizing the installed shoulder harness restraint system. The aircraft sustained substantial damage. This accident is closed.
