From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) While herding wild horses with his helicopter, pilot went into a hover at about 40 feet above the ground. All of a sudden the engine wound up to a high RPM and the main and tail rotors became unpowered. The pilot tried to auto rotate to a field but only had time to pull in collective. He hit the ground at a high rate of descent. The helicopter was substantially damaged. The helicopter was taken to the operators main base. A detailed inspection of the helicopter transmission and associated parts will be conducted by FAA airworthiness inspectors and the operator. If a failure of any part is discovered an m or d will be filed.
