From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. (-23) On Friday, April 16th, 2004, approximately at 12:30PM Alaska daylight saving time, Piper PA-12 (N3029M s/n 12-1720) airplane was involved in an accident. The aircraft was being operated by the pilot as a VFR flight under title 14, CFR part 91, when the accident occurred conditions were ideal for icing. Evaluation of runway indicated three large puddles with high grass and mud with six inches of water that the pilot rolled over. The second puddle hit caused the water to splash up to the windshield and wash on the front of the aircraft. The pilot continued with the takeoff. The pilot decided not to abort, but continued until he was in the air 50 feet. He turned right. The engine slowly died and aircraft crash landed. Minor injuries. The aircraft received substantial damage to landing gear, right wing and prop/engine.
