From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Helicopter N6291T, registered to privacy data omitted, and operated under 14 CFR part 141 by hillsboro helicopters, inc., 3565 NE cornell rd, hillsboro, or 97124, sustained substantial damage at approx 19:00 on may 5, 2004. The helicopter made an emergency autorotation landing into a field 1/4 mi. West of Chehalis airport (cls) after the engine quit as the helicopter initiated a left turn entering the downwind leg for runway 33. The helicopter touched down tail first in a left yaw. The right skid collapsed causing the helicopter roll over on it's right side. The rotor blades struck the ground on the right side and came in contact with the ground only. External visual inspection revealed that damage to the helicopter resulted from stress occuring at the right skid, tail boom and rotor mast attach points as a result of the hard landing. There was no fire and the flight instructor and student were not injured. During the salvage operation, the engine started easily and ran normally. The helicopter is being transported to Robinson helicopter co. for further investigation. No cause for loss of power was evident at the scene.

(-23) HELICOPTER N6291T, REGISTERED TO ^PRIVACY DATA OMITTED^, AND OPERATED UNDER 14 CFR PART 141 BY HILLSBORO HELICOPTERS, INC., 3565 NE CORNELL RD, HILLSBORO, OR 97124, SUSTAINED SUBSTANTIAL DAMAGE AT APPROX 19:00 ON MAY 5, 2004.
