The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
(-23) On 7/14/04, at approximately 1700E aircraft t/o from alb enroute to iad. Though 13,000 MSL crew observed lav smoke and ice protection indication on eicas. Then notified ATC of the situation. At 1721E, aircraft returned to land on rwy 01. ARFF responded and the aircraft taxied to the ramp without incident. On 7/15/04, at 0600, raia mechanic inspected and performed an operational check the environmental system. No discrepancies were discovered. On 7/15/04, at 0530, FAA inspector arrived at the aircraft to investigate the incident. After a thorough review of the aircraft log, it was observed by the inspector there were two previous entries in the aircraft log relating to the incident. Discrepancies are as follows: "hot air coming out of refuel panel while #2 pack is on" corrective action: "resecured clamp i/a/w 145 m/m 21-51-01". Second discrepancy as follows: "pack #2 side had hot bulkhead near and around fuel panel" corrective action: "deferred #2 pack hot air leak i/a/w MEL 21-51-00 cat c". In addition, as a result of the incident, inspector observed a discrepancy in the log book describing a hard landing. Correction block states as follows: "crew flew within 360 feet per second sink rate and no inspection is required". If corrective action information is accurate that would mean 21,600 feet per minute sink rate. Obviously the corrective action block contains an incorrect entry. Therefore, it is possible that a documented over weight landing inspection may have been required. On 07/15/04, aircraft was returned to service and departed without incident. privacy data omitted On 07/15/04 pmi was notified of inspector's concerns.
As the FAA filed it
(-23) ON 7/14/04, AT APPROXIMATELY 1700E AIRCRAFT T/O FROM ALB ENROUTE TO IAD. ^PRIVACY DATA OMITTED^ ON 07/15/04 PMI WAS NOTIFIED OF INSPECTOR'S CONCERNS.
