From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) During the after take off climb, the flight crew noticed a vibration accompanied by an unusual whining sound that did not change in amplitude after the crew retarded power. The eicas displayed a level 5 vibration msge for the l engine, but no fire warnings. Fumes entered the cockpit and cabin causing the flight crew to don oxygen masks, declare an emergency and immediate return to the airport of departure. The aircraft made an overweight landing on runway 15L and exited at taxiway W-3. The ARFF reported a ball of fire in the lower (7 o'clock) portion of the l engine exhaust nozzle. When the flames increased, aqueous film forming foam (afff) was applied, extinguishing the flames. When the crew was advised of the fire, the captain ordered an evacuation of the aircraft. The flight crew shut down the engines and exited the aircraft.

As the FAA filed it
