From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Mes flight 3690 departed msp on 10/24/2004 at 0800. The pilot not flying (captain) failed to set the pressurization system for takeoff, climb, and cruise. The aircraft did not pressurize causing the oxygen masks to automatically deploy at 13,500 feet MSL. The aircraft returned to msp and landed without further incident. Captain and first officer were removed from line flying. A check was given by a mesaba airlines, inc. check pilot on October 31, 2004. Both pilots failed the check, were put into a training program of additional ground school, and simulator training. A proficiency check was given to both pilots, which were passed and the pilots returned to line flying. All checks were observed by FAA inspectors.
