From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) On January 28, 2005, at approximately 1625 MST, the CFI privacy data omitted was providing primary flight instruction to the student pilot privacy data omitted in a Robinson R-22 beta helicopter (N7535Z). As the upslope skid made contact with the ground, both pilots felt the aircraft begin to settle aft. privacy data omitted ^ Stated that he assumed the flight controls and applied up collective to lift the aircraft off from the slope. As the aircraft cleared the slope, the aircraft began to spin to the right. The aircraft made one to two turns to the right and then contacted the ground in an extreme nose low attitude. The aircraft continued to drift to the left in rotation until the left skid contacted the ground. The aircraft rocked up on the left skid and both main rotors contacted the ground. The aircraft became airborne again drifting aft. The aircraft came to rest upright. The crew performed normal shutdown of the engine and egressed the aircraft unassisted. The aircraft sustained substantial damage to the fuselage, main rotors, tailboom and skid assemblies.
