From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Pilot attempted to take off on runway 36 at Morris, IL (C09) airport. Aircraft started to rotate for take off at approximately 50 mph. Pilot claims he was strapped into seat (shoulder harness) and pilot seat slipped back on the seat rails to full aft position and he could not reach any flight controls. The aircraft settled to the ground, then departed to the left side of runway. It then re-entered the runway severing the landing gear. It skidded to a stop about one-third of the way down the runway. A taxiway light was broken by the aircraft. A side load on both landing gear collapsed the gear toward the right side. Damage was incurred on the right wing from the landing gear punching up from the bottom.
