From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

According to Pueblo air traffic control tower, the helicopter had been conducting multiple approaches to runway 8R and had been cleared for the option. One witness stated that the helicopter appeared to be in a steeper approach than the previous approaches. The helicopter impacted the runway hard, splitting both landing skids out laterally. It rolled over on it's left side and caught fire. All three rotor blades were bent, the tail boom separated at the tail rotor drive shaft, and the tail rotor blades were fragmented. The fuselage was crushed and wrinkled and the plexiglas bubble fragmented.
