From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) An as 350 with 6 passengers onboard crashed in boggy terrain while departing Juneau airport (jnu), on Monday, July 25, 2005, at approximately 2005Z. Aircraft had substantial damage, no injuries to all onboard. Accident occurred at temsco helicopters heliport, Juneau intl. Airport. This flight was a 14 CFR part 135 flight with passengers for sightseeing to the glacier. The aircraft was operated one short flight prior to accident flight to take parts/mechanic up on the glacier to tend to another aircraft. That pilot reported no problems with aircraft. It appears that after takeoff the turbine blade or blades came apart in the engine and smoke/fire was observed from persons on the ground. This happened just after the initial liftoff from the heliport and attaining about 100 feet altitude. Amendment to follow with engine teardown report from manufacturer.
