What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
(-23) On August 11, 2005 at about 1215 eastern daylight time, a Robinson R-22-beta, N8320A registered to privacy data omitted and operated by the pilot privacy data omitted crashed during an autorotation at klna. VMC conditions prevailed at the time of flight for the 14 CFR part 91 instructional flight. The helicopter incurred substantial damage. There were no injuries reported. The flight originated from klna. A written statement from the pilot in command indicated that they were practicing straight in autorotations to taxiway b located on the right side of runway 15. They entered the maneuver at 700 feet by putting the collective down, adding right pedal, and aft cyclic with the helicopter level and then the needle split (rotor tachometer). They established the autorotation at 100 knots with the throttle at the detent. (This was accomplished because the student was a CFI candidate). The flare was entered at 40 feet and 60 knots. The wind blew the helicopter away from the taxiway centerline. At this point the instructor pilot called for a recovery. The throttle was brought back up from the detent and power was continuously added until the helicopter touched the ground. The left skid touched down outside of the taxiway in the grass. The low rotor RPM horn came on. The helicopter hit hard on the left side. The instructor pilot was at the controls after the recovery was called for. During the entire maneuver the RPM (rotor) was said to be at 100-102%.
