From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Pilot loaded an eighty pound bag of cement into the passenger seat to evaluate aircraft performance with additional weight. During takeoff pilot noticed that the aileron flight controls were binding and becoming very stiff. Pilot lost control of aircraft approximately 30 feet in the air and the left wingtip hit the runway followed by the left main landing gear which failed. Prop hit runway and shattered. Tailwheel attaching bolts failed and the rudder hit the runway. Aircraft completed a 180 degree turn on the runway. Substantial aircraft damage and no injuries to the pilot. Seat pan distorted during the takeoff roll and caused the aileron push-pull tubes to bind under the pilot's seat.
