The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
(-23) On October 22, 2005, at approximately 1630 MST, N22532, an experimental Nord 3202 airplane, with the pilot privacy data omitted performing as pilot-in-command, crashed while performing touch-and-go landings at Casa Grande Municipal Airport. privacy data omitted A student pilot, sustained fatal injuries. The pilot privacy data omitted had been performing touch-and-go landings in the Nord with his father, privacy data omitted, a commercial pilot. privacy data omitted Provided a written statement that he and his son the pilot ^priva^ had been practicing touch-and-go landings and that after 10-15 landings, privacy data omitted got out of the airplane to let the pilot ^priva^ continue to perform solo touch-and-go landings. After performing approximately 5 additional landings, privacy data omitted states that the pilot ^priva^ was making a turn from base to final for runway 5 and encountered a twin engine airplane also on final after completing a practice ILS approach. The pilot ^priva^ maneuvered his aircraft to avoid contact with the twin and entered a 60 degree bank to the (left). The aircraft entered a stall and spun approximately 180 degrees impacting the ground at a high rate of descent on a heading of 220 degrees. The aircraft slid approximately 20 feet to the west and came to rest on a heading of 205 degrees. privacy data omitted Statement indicates that he was taking pictures of the pilot ^priva^ performing touch-and-go's, however does not indicate whether or not he has a picture of the twin that the pilot ^priva^ encountered on final. privacy data omitted Has refused to speak with investigators of the FAA and NTSB, and has so FAR only provided a short written statement. There were no other witnesses located at or near the airport that observed the accident, however, several stated that they saw the privacy data omitted aircraft in the traffic pattern and saw privacy data omitted and his other son ^priva^ near the airport FBO. Witnesses did not see the twin or near mid-air with the pilot's privacy data omitted aircraft. This investigation has revealed that the pilot's privacy data omitted student pilot certificate expired in March 2003, and that his medical expired March 2004. The privacy data omitted have so FAR refused to submit the pilot's privacy data omitted pilot logbooks for review. privacy data omitted Is not a certified flight instructor. There is no evidence that the pilot privacy data omitted has flown with a flight or obtained the necessary endorsements to perform solo flight. privacy data omitted Medical certificate expired in October 2002. There is no record of privacy data omitted attempting to obtain another medical certificate. N22532 is not currently legally registered. privacy data omitted Acquired this aircraft in may, 1995, and was advised in February 1998, by aircraft registry in Oklahoma city, that she needed to submit an aircraft registration application. This was never accomplished. Current registration indicates "sale pending." The autopsy of the pilot privacy data omitted indicated that he died of massive blunt trauma to the face. Placement of the shoulder harness and damage to the instrument panel in the aircraft indicated that the pilot ^priva^ was not wearing his shoulder harness at the time of the accident. The coroner's report substantiates that the pilot ^priva^ did not have any marks that would indicate that he was wearing the shoulder harness. privacy data omitted, Who was the first to arrive at the scene of the accident, has made statements indicating that the pilot ^priva^ was not wearing the shoulder harness.
As the FAA filed it
(-23) ON OCTOBER 22, 2005, AT APPROXIMATELY 1630 MST, N22532, AN EXPERIMENTAL NORD 3202 AIRPLANE, WITH THE PILOT ^PRIVACY DATA ^ PERFORMING AS PILOT-IN-COMMAND, CRASHED WHILE PERFORMING TOUCH-AND-GO LANDINGS AT CASA GRANDE MUNICIPAL AIRPORT. ^PRIVACY DATA OMIT^ A STUDENT PILOT, SUSTAINED FATAL INJURIES. THE PILOT ^PRIVACY DATA OMIT^ HAD BEEN PERFORMING TOUCH-AND-GO LANDINGS IN THE NORD WITH HIS FATHER, ^PRIVACY DATA OMI^, A COMMERCIAL PILOT. ^PRIVACY DATA OMI^ PROVIDED A WRITTEN STATEMENT THAT HE AND HIS SON THE PILOT ^PRIVA^ HAD BEEN PRACTICING TOUCH-AND-GO LANDINGS AND THAT AFTER 10-15 LANDINGS, ^PRIVACY DAT^ GOT OUT OF THE AIRPLANE TO LET THE PILOT ^PRIVA^ CONTINUE TO PERFORM SOLO TOUCH-AND-GO LANDINGS. AFTER PERFORMING APPROXIMATELY 5 ADDITIONAL LANDINGS, ^PRIVACY DAT^ STATES THAT THE PILOT ^PRIVA^ WAS MAKING A TURN FROM BASE TO FINAL FOR RUNWAY 5 AND ENCOUNTERED A TWIN ENGINE AIRPLANE ALSO ON FINAL AFTER COMPLETING A PRACTICE ILS APPROACH. ^PRIVACY DATA ^ STATEMENT INDICATES THAT HE WAS TAKING PICTURES OF THE PILOT ^PRIVA^ PERFORMING TOUCH-AND-GO'S, HOWEVER DOES NOT INDICATE WHETHER OR NOT HE HAS A PICTURE OF THE TWIN THAT THE PILOT ^PRIVA^ ENCOUNTERED ON FINAL. ^PRIVACY DAT^ HAS REFUSED TO SPEAK WITH INVESTIGATORS OF THE FAA AND NTSB, AND HAS SO FAR ONLY PROVIDED A SHORT WRITTEN STATEMENT. THERE WERE NO OTHER WITNESSES LOCATED AT OR NEAR THE AIRPORT THAT OBSERVED THE ACCIDENT, HOWEVER, SEVERAL STATED THAT THEY SAW THE ^PRIVACY D^ AIRCRAFT IN THE TRAFFIC PATTERN AND SAW ^PRIVACY DAT^ AND HIS OTHER SON ^PRIVA^ NEAR THE AIRPORT FBO. WITNESSES DID NOT SEE THE TWIN OR NEAR MID-AIR WITH THE PILOT'S ^PRIVACY DATA OM^ AIRCRAFT. THIS INVESTIGATION HAS REVEALED THAT THE PILOT'S ^PRIVACY DATA OM^ STUDENT PILOT CERTIFICATE EXPIRED IN MARCH 2003, AND THAT HIS MEDICAL EXPIRED MARCH 2004. THE ^PRIVACY D^ HAVE SO FAR REFUSED TO SUBMIT THE PILOT'S ^PRIVACY DATA OM^ PILOT LOGBOOKS FOR REVIEW. ^PRIVACY DATA OMI^ IS NOT A CERTIFIED FLIGHT INSTRUCTOR. THERE IS NO EVIDENCE THAT THE PILOT ^PRIVACY DATA ^ HAS FLOWN WITH A FLIGHT OR OBTAINED THE NECESSARY ENDORSEMENTS TO PERFORM SOLO FLIGHT. ^PRIVACY DATA ^ MEDICAL CERTIFICATE EXPIRED IN OCTOBER 2002. THERE IS NO RECORD OF ^PRIVACY DAT^ ATTEMPTING TO OBTAIN ANOTHER MEDICAL CERTIFICATE. N22532 IS NOT CURRENTLY LEGALLY REGISTERED. ^PRIVACY DATA^ ACQUIRED THIS AIRCRAFT IN MAY, 1995, AND WAS ADVISED IN FEBRUARY 1998, BY AIRCRAFT REGISTRY IN OKLAHOMA CITY, THAT SHE NEEDED TO SUBMIT AN AIRCRAFT REGISTRATION APPLICATION. THIS WAS NEVER ACCOMPLISHED. CURRENT REGISTRATION INDICATES "SALE PENDING." THE AUTOPSY OF THE PILOT ^PRIVACY DATA ^ INDICATED THAT HE DIED OF MASSIVE BLUNT TRAUMA TO THE FACE. ^PRIVACY DAT^, WHO WAS THE FIRST TO ARRIVE AT THE SCENE OF THE ACCIDENT, HAS MADE STATEMENTS INDICATING THAT THE PILOT ^PRIVA^ WAS NOT WEARING THE SHOULDER HARNESS.
