From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) After initiation of t/o roll Chicago o'Hare tower advised ual #394 that one (1) nose gear was missing, the aircraft aborted t/o at approximately 60 knots. The flight crew write-up was: tire came off when finishing turn onto runway 9R. Heard loud thump. Aborted takeoff below 60 knots with no abnormal or excessive side loads. Maintenance inspection revealed the left nose wheel had separated from the aircraft's NLG inner cylinder. Further investigation revealed two previously undetected cracks in the left side axle of the NLG inner strut. NLG removed and forwarded to uala's quality and engineering for evaluation. United airlines FAA certificate management office is following investigation.
