From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Aircraft departed phx on a part 135 flight. On climb out, nose gear failed to retract leaving nose gear unsafe indication. Crew cycled gear handle but failed to get safe indication. During the recycling, the crew reported hearing a thump/catching noise from the front of the aircraft. Following the emergency procedures checklist, the crew pulled the emergency gear extension knob. Crew reported hearing the nitrogen bottle discharge, but the gear indication remained the same. Crew notified ATC of emergency and was vectored to phx runway 8. Normal let down and main wheel contacted pavement, yoke was used to hold the nose wheel off. As aircraft slowed, nose gear collapsed allowing the nose of the aircraft to contact the pavement. After stopping, crew shut all systems down, secured aircraft and exited the aircraft through the main cabin door.
