From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

(-23) Received call at approx 11:30 pm from ifo mgmt concerning a turkish airline A-340 aircraft that skidded off rwy 31L upon landing. Contacted the com center for a briefing on the status of the aircraft. Met ops inspector at nyifo and continued to papd for access to rwy. Upon arrival at the scene noticed a/c had skidded completely to the left of the rwy with approx 110 deg turn to the left. All aircraft landing gear was off the pavement and buried to the top of the wheels on the outboard side in the direction of the skid with snow and dirt. Also noticed that the #2 engine still had all its t/r elephant ears open. Yet all 3 other engines had t/r's stowed. Papd took pictures. When turkish maintenance and station manager arrived, they were asked as to whey the #2 engine was still in reverse. They were uncertain, yet engine throttles in cockpit were all retarded. Received a copy of the motor vehicle accident report from the port authority police dept. Additional ptrs comment: aircraft landed without incident on 31L jfk, when thrust reversers were retracted the left inboard reverse failed to retract. The aircraft pulled to the left and captain was unable to keep the aircraf t on the runway. Aircraft came to rest with nose facing approximately 100 degrees from center line and 100 feet from the runway edge. The captain had no cockpit indication that the reversers were still deployed. Passengers were deplaned with the aid of papd. There were no injuries reported. The captain and crew were interviewed and concurred with the above. Five (5) turkish airline airworthiness personnel were dispatched from turkey to evaluate the problem. It was determined that a faulty signal was sent to the reverser. The problem was corrected and aircraft returned to turkey.
