From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) The pilot was performing a maintenance check flight following repairs and an annual inspection. During approach to foe when the landing gear was selected "down" the nose gear down light (green) did not illuminate. The gear was cycled 2 more times with the same results. Emergency procedures were followed. A request was made to ATC for a flyby for a visual on the gear. The gear appeared to be down. Upon landing at foe and during rollout the pilot stated as the nose of the aircraft settled down on the nose gear it collapsed. This resulted in the propeller striking the ground and the aircraft skidded to a stop. No injuries were reported.
