From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Witness statements provided information that N183AE was conducting a series of takeoff and landings on the taxiway at 5T6 (at least 4 or as many as 6 in sequence). Additional information from oas investigation team member (not a witness) indicates that transition training for mission aircraft was being conducted. Ip was providing training to his first candidate after completing ip training. Ip may have been conducting emergency (manual) governor operations to landings. Information provided to FAA indicates that a maintenance alert was issued in 2004 against this model to limit manual operation of governor due to potential for overheating and possible failure of solenoid. If solenoid were to fail in low-idle mode, pilot may be unable to prevent rotor RPM decay. Maintenance alert permitted two applications of manual governor operation in 15 minute period followed by 15 minutes suspension of operation.
