From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Taxied onto runway 13 in preparation for takeoff. Performed all preflight runup checks. Temp about 90, wind ssw @ about 7-9 mph, no clouds, clear. Advanced throttle for takeoff. At about 35 mph lifted the tail by pushing the stick fwd. The plane lurched to the left even with r/h rudder applied. The plane went on grass to the left of the runway. Immediately cut the throttle. Became aware of a drainage ditch ahead so pulled back on the yoke in an effort to keep tail down. Approx speed 20-25 mph. The front wheels bounced traversing 1st trench. The tail wheel hit the tranch and tail flipped up and the plane went over on its back.
