(-23) On 07/19/2006 at approximately 0930 CDT the home of privacy data omitted was struck by a large piece of ice estimated to weight in excess of ten pounds. On scene investigation by this inspector and inspector privacy data omitted determined that the ice had fallen from a passing aircraft. From the color and clarity of the ice that had been preserved by privacy data omitted it was determined that the ice came from the aircraft potable water system. The ice penetrated the roof and ceiling of privacy data omitted home striking a television set in the family room of the house. There were no injuries to occupants of the home. A review of the radar plot provided to this office by rfd approach control indicated four jet aircraft in the vicinity of the privacy data omitted home at the time of the incident. Three of the four aircraft were air carriers operating under CFR 14 part 121 and one was privately owned operating under CFR 14 part 91. An inspection and review of maintenance records on all four aircraft revealed that only one aircraft was found to have a leak in the drain and servicing point of the potable water system at the time of the incident. The facts lead to the conclusion that the leaks in thepotable water system allowed ice to form on the exterior of the aircraft at cruise altitude. As the aircraft descended into warmer air the ice separated from the aircraft and struck the privacy data omitted home. American eagle flight 354 was operating in the vicinity of the privacy data omitted home at 19,000 ft at 0922 CDT. On 07/20/2006 this aircraft was found on inspection to have leaks at the drain and service point of the potable water system. There are no eye witnessed to verify the possible cause of this incident beyond the investigators conclusions which are based on circumstantial evidence.
