From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) While conducting flight instruction in a Robinson R-22 helicopter the instructor performed a slope landing. The instructor stated that the slope was too steep, so he decided to level off and lift off of the surface. Upon liftoff he believed that the left skid hung which caused the helicopter to roll over causing substantial damage. The pilot received minor injuries and the student was not injured. After meeting with the pilot he has agreed to surrender his rotorcraft instructor rating and this certificate has been replaced with a temporary student pilot rotorcraft certificate until such time that he can demonstrate his performance in the helicopter with an FAA operations inspector.
