From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) On 2-28-07 pilot examiner, privacy data omitted was giving training and instructional flights to 2 other pilots in N330KW, a SA-330-J puma (with a retractable landing gear) helicopter for single engine emergency landings, when he failed to extend the landing gear on landing and rested the helicopter on its belly. The instructor elected to hover the helicopter, extend the gear and shut it down to ascertain the damage. There were no injuries to the pilot or the other 2 crewmembers onboard. Upon inspection of the helicopter belly, he noted the cargo hook had been shoved into the aft bulkhead under the belly, into the area of the left transverse fuel tank. The pilot failed to have a certified mechanic inspect this helicopter for airworthiness and elected to fly this helicopter 4 miles to the hangar facility. (-23 amendment) on 2-28-07 pilot examiner, privacy data omitted was giving training and instructional flights to the other pilot in N330KW, a SA330-1 puma (with a retractable landing gear) helicopter for single engine emergency landings, when he failed to extend the landing gear on landing and rested the helicopter on its lower fuselage section. The instructor elected to hover the helicopter, extend the gear and shut it down to ascertain the damage. There were no injuries to the pilot or the other pilot onboard. Upon inspection of the helicopter lower fuselage section, pilot noted the cargo hook had been shoved into the aft bulkhead under the lower fuselage section and into the area of the left transverse fuel tank. The pilot failed to have a certified mechanic inspect this helicopter for airworthiness and elected to fly this helicopter 4 miles to the hangar facility, without obtaining a special flight permit from the FAA.
