From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) PIC reported the tail rotor pedals quit functioning during flight. Declared an emergency and returned to ogg airport. Able to land with little or no forward speed. Aircraft spun around twice before touchdown. Inspection revealed the tail rotor gear box upper attachment fitting cracked about 2 inches from bellcrank bolt. The t/r gear box had 111.9 hours before overhaul with a total time of 3188.1 hours. The only damage is on the t/r gear box attachment fitting, there is no "aircraft" damage. The t/r gear box has been submitted to NTSB in California. Tail rotor gear box will be analysed by NTSB, FAA, and eurocopter-france to determine failure. The director of maintenance has submitted an m & d report of damage.
