From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Crew aborted first take-off run on runway 22 at approximately 100 knots when the right thrust reverser system alerted and illuminated the caution light for thrust reverser unlocked. The bleed air and deployed lights did not illuminate during the take-off run, so the crew cycled the thrust reverser system several times during the taxi back and confirmed visually that the right reverser was stowed shut and all the lights were working properly. They made a second take-off attempt but aborted at 50 knots when the same caution light illuminated. During the second abort, the main tires failed sequentially and there was wheel rim and brake damage to the aircraft. A micro switch was replaced on the right thrust reverser. The manufacturer has simplified the thrust reverser advisory system in subsequent aircraft and it is more effective and robust. Crew was counseled on the need to cool the tires after a long taxi or a take-off abort.

As the FAA filed it
