From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) This incident involves a skydiver privacy data omitted conducting parachute jumping activity under 14 CFR part 105. Witness statements indicate that privacy data omitted exited the aircraft at approximately 12,500 MSL. One witness statement indicated that the main parachute was deployed at a lower than normal altitude. Several statements indicate that upon main parachute deployment, privacy data omitted experienced a "line twist" where several parachute shroud lines were tangled. One statement indicated that privacy data omitted was observed kicking his legs, perhaps in an attempt to maneuver the parachute and correct the line twist. Another witness stated privacy data omitted hands dropped to his sides, suggesting that he may have pulled both toggles down. According to a uspa safety official, this action can result in "cross controlling/control reversal" caused by the twisted lines. Subsequently, several witnesses indicate that the chute began spinning uncontrollably, causing his rate of descent to increase. Witnesses statements indicate that he impacted the ground near a wooded area just west of the landing zone at nichols field. privacy data omitted Was fatally injured. The NTSB did not investigate.

ONE WITNESS STATEMENT INDICATED THAT THE MAIN PARACHUTE WAS DEPLOYED AT A LOWER THAN NORMAL ALTITUDE. SEVERAL STATEMENTS INDICATE THAT UPON MAIN PARACHUTE DEPLOYMENT, ^PRIVACY DATA OMITTED^ EXPERIENCED A "LINE TWIST" WHERE SEVERAL PARACHUTE SHROUD LINES WERE TANGLED. ANOTHER WITNESS STATED ^PRIVACY DATA OMITTED^ HANDS DROPPED TO HIS SIDES, SUGGESTING THAT HE MAY HAVE PULLED BOTH TOGGLES DOWN. ^PRIVACY DATA OMITTED^ WAS FATALLY INJURED. THE NTSB DID NOT INVESTIGATE.
