From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) After a normal approach and landing, collective was lowered to flat pitch when a severe vertical vibration occurred. Emergency shut down procedures were employed. After exiting the helicopter found star flex had separated at blue blade and tailboom had crush damage aft of connection point. Separation of star flex appears to be "static overload." Complete assembly being sent to Eurocopter france for evaluation. Component total time 481.6. AD 2000-20-19 is applicable and was complied with at component time of 55.0 hrs. Operator inspected remaining fleet of 37 AS-350 helicopters and found no anomalies.
