From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) The aircraft doors were closed and the front end crew and the flight attendant crew were preparing for the pushback from the gate at kmdw. The final paperwork for the flight was in the cockpit with the signed release. This release was supposed to have been given to the gate agent prior to closing the IL door. The captain realized this and asked the a flight attendant to open the door and give the paperwork to the gate agent. Contrary to frontier procedures, the a flight attendant opened the "armed" door and the slide pack deployed but did not inflate because it fell against the jetway which was still in place. This incident was an inadvertent slide deployment made possible because company procedures were not followed. privacy data omitted

As the FAA filed it

^PRIVACY DATA OMITTED^
