From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Aircraft landed on runway 27. Once parked, maintenance personnel noticed a five foot hole in the inboard side of the number three engine thrust reverser transclating cowl. A lower inboard thrust reverser translating cowl actuator rod-end had broken. The broken actuator began banging around when the thrust reverser was deployed on landing and hit the translating cowl causing a hole. This hole, when combined with the wing generated by the thrust reverser deployment, caused the composite translating cowl to break away and finally end up with a five foot hole in the translating cowl.
