From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Flight was an introductory ride for a new student in the DA-40 aircraft. After preflight and taxi out, instructor and student completed the before takeoff checklist and took off for traffic pattern familiarization. During initial climb after takeoff, the door open light illuminated with an aural tone. On downwind, the instructor checked the canopy and passenger door for security. The passenger door was ajar. On final approach, the instructor attempted to pull door closed, but the door opened into the slipstream. The instructor took the flight controls and landed the aircraft without further incident. After stopping aircraft, the door was found to have departed the aircraft on short final. The door was recovered just short of the runway by airport fire personnel.
