From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) At FL380 crew reported the cabin altitude horn went off. Cabin altitude was above 1000', could not re-establish pressure. Crew executed emergency descent, reporting the right pack at 5 PSI and the left at 40 PSI. All door warning lights showed closed. After descending below FL120 crew reported the right pack returned to 40 PSI. The crew landed without incident at cle. Cala. Cle maintenance found the problem was due to low flow from the NR2 bleed air system. The fire handle switch was found faulty causing the pressure regulator shutoff valve to close which caused low inflow to the cabin. The fire control panel (P8-1) and the high stage regulator was removed and replaced. End
