The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
N839QS flew from sugarland, TX (ksgr) to Telluride, CO (ktex) on September 1, 2008. The flight landed at approximately 17:14 local time. The aircraft touch down at approximately 1,000 feet down on runway 9, which is congruent with aircraft debris found near 1,500 feet on runway 9. The debris found at that location was shredded tire pieces and nose gear rim pieces. At approximately 3,000 feet down runway 9 skid marks from the nose gear fork started near the left edge of the runway. At that point the aircraft went off the runway and into the grassy area to the left side of the runway. The left wing and the nose of the aircraft sustained damage from hitting runway edge lights and the runway distance remaining sign (4,000 foot remaining sign for runway 27). The left aileron sustained substantial damage from impacting the runway lights. The aircraft paralleled the runway just to the left of the runway edge lights. The aircraft came to rest on the east side of A2 taxiway. The main gear was down and locked resting next to the east side of the pavement on A2 taxiway. The nose gear was folded backwards and under the nose of the aircraft. The aircraft came to rest sitting on its nose. There were no injuries to the pilot and co-pilot. Thrust reversers were deployed by the pilots at some point in the landing phase and remained deployed when the aircraft was shutdown. The engines did ingest some dbris and sustained damage to the turbine blades. The gear was bent backwards and did damage the pressure vessel. The trunnion bolts were intact. The shimmy dampener bolt was sheared. The nose gear actuator was bent but still intact. The scissors was turned 180 degrees but still intact. The nose fork was ground down and the wheel assembly was missing. The axel through bolt was found on the runway at approximately 3,000 feet. The axel through bolt was bent, the castle nut hole was intact, and the threads were sheared off. At 16:46 local the weather conditions at ktex were VFR with winds from the southwest at 3 knots. Clouds were scattered at 6,000 and broken at 8,500. The temperature was 63F, dew point 39F. The altimeter setting was 30.21. The maintenance logs showed compliance. On 08/04/2008 the nose wheel assembly was removed and replaced iaw CE560 mm 32-41-02. This repair was due to a write up form for a “nose wheel vibration on lift off.” also listed in the corrective action was “inspected nose gear torque links for excessive play and lubricated, no defects noted. Replaced nose wheel/tire assy ref other mzb on this work order. Nose wheel shimmy dampener was check for proper service, OK.
As the FAA filed it
N839QS FLEW FROM SUGARLAND, TX (KSGR) TO TELLURIDE, CO (KTEX) ON SEPTEMBER 1, 2008.
