From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) During takeoff from runway 27 (length 3,998 feet) at Wallace, NC airport (acz), pilot crashed N21MY into a pine forest off the departure end of runway 27 around 10:30 am. After liftoff (approximately 50 feet above runway and just short of midfield) engine lost power, operated at reduced power, and would not climb. Aircraft stayed in ground effect for remainder of runway and gained no more than approximately 60 to 80 feet with landing gear down prior to settling into the top of a pine forest, approximately 968 feet from runway. Post crash investigation revealed throttle, mixture and propeller controls in the full forward positions. There was no sign of air inlet plug and lanyards in aircaft. Inspection of engine after crash revealed red lanyard material (tpe that is used for streamers for ram air inlet plugs) lying on top of rear engine cylinders. Remains of the red lanyard material was also found around and behind the propeller. Engine failed to produce full power because ram air inlet plug material lodged in front of the air filter.

As the FAA filed it
