From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) 09/11/08 At approximately 1625, flight instructor privacy data omitted while providing dual inspection to privacy data omitted, crashed a Robinson R-22 aircraft. During the attempt to execute a 180 auto rotation, the aircraft lost airspeed and rotor RPM. A very late attempt was made to initiate a go around or recovery. The aircraft contracted the runway hard. The tail rotor separated and the aircraft proceeded to rotate violently for several rotations until coming to rest in the dirt on its side. Minor injury was sustained to privacy data omitted only. Inspection of on board aircraft documents indicate a conflict with the airworthiness certificate and registered aircraft tail number. Certificate reflects the original owner, silver state helicopter N470SH. The painted tailed number reflects the guidance helicopter new owner N227GH. Updated registration missing. Aircraft registration branch letter, acknowledging the new owner requested change only.
