(-23) On March 7, 2009 at approximately 4:45 pm a tbm, NL188TD, departed runway 14 at the Millville Municipal Airport (miv) for a test flight. The aircraft previously had the carburetor overhauled and reattached. There were several "runups" conducted with no problems. The aircraft then taxied to runway 14 for departure. The aircraft departed without incident. As it was climbing several eyewitnesses observed smoke, dark in color, started to develop from the engine cowl (lower right side). Shortly thereafter, flames could be seen from the cowling. The aircraft, piloted by privacy data omitted, then made an immediate turn to runway 28 for landing. On final, the aircraft struck a small tree and split it vertically. The aircraft continued its flight path towards runway 28, dropping metal parts and possible fuel. A brush fire developed along part of the aircrafts flight path. The aircraft landed on the grass short of the runway it continued paralleling the runway for approximately 1100 feet. The aircraft then rolled onto the runway and continued for 1248 feet. The aircraft was observed fully engulfed in flames, the pilot had bailed out of the aircraft and landed onto the runway. (Cont'd). privacy data omitted Has second and third degree burns and has smoke inhalation. He is presently induced in a coma and on ventilation.
